Separate Account Reference sheets are permitted but you must still complete the 1st, 2nd and 3rd sections of this sheet and sign for our records. Please make sure you have net terms with your listed account references and that you have supplied their fax numbers or your application cannot be processed.

- Credit Card accounts can take up to 72 hours.
- Net 30 accounts may take longer (depending how long it takes for your references to respond).
- We will email you a welcome letter with your account number and pricing sheet/order form.
- RoomMates accounts will be given access to all product images via our online image database. Your username and password for the database will be provided upon completion of account setup.
